How to Apply

Once the requirements are in place take the following two steps:

1. Complete the application form which can be submitted online.

2. Print off the Data Protection Agreement for GP Practices or the Data Protection Agreement for Agencies, sign it and return to us by fax (01 830 7728) or post. When we receive your forms we will set up your Healthlink account and contact you to arrange a date for set-up and training.
